Follow procedure below to complete.
To Sign-off for a meet:
1. Please fill out the form below
2. To sign off for a Saturday dual meet, you must do so by Friday, one week prior to the dual meet you will be missing. Lineups are made one week before the meet on Saturday. In other words... Coach Sue needs to know if you will NOT make a meet ...one week in advance!
3. If you know you will be out of town, submit dates you are unavailable as soon as possible so that coaches can plan ahead.
4. You must hit the "Submit" button.
5. You must receive an e-mail response back from Coach to confirm that your notice was received.
